The mining industry is one of South Africa’s highest-risk working environments, with employees regularly exposed to physical, chemical, biological, and ergonomic hazards. From underground operations to surface mining, workers may face risks such as dust exposure, noise, vibration, heavy machinery, confined spaces, heat stress, and fatigue — making occupational health medicals essential for safety and legal compliance.

Occupational health in the mining sector is more than a regulatory requirement — it is a shared responsibility that protects employees, contractors, and mining operations. Strict legislation requires that all mine workers, including short-term contractors, be medically fit to perform their duties safely. Pre-employment, periodic, and exit medical examinations — commonly referred to as Red Ticket medicals — play a vital role in preventing occupational disease, workplace injuries, and long-term health complications.

Employee health has a direct impact on mine safety, productivity, and compliance with legal requirements. Regular occupational health medical surveillance and examinations help ensure employees are medically fit for their specific job roles — both before employment and throughout their employment, as well as upon exit. To support a risk-based and legally compliant medical assessment, the Employer must complete a Man Job Spec Form, detailing the employee’s job-specific risks and exposures, and submit it together with the Employee/ Patient at the time of the occupational health medical examination.

Industry regulations that determine which medicals to complete include:

  • Occupational Health and Safety Act, No. 85 of 199
  • Mine Health and Safety Act (MHSA), No. 29 of 1996
  • Regulations for Hazardous Chemical Agents, 2021
  • Noise-Induced Hearing Loss Regulations, 2003
  • Lead & Heavy Metals Regulations
  • Silicosis & Tuberculosis (TB) Medical Surveillance
  • Radiation Protection Regulations

Follow-up medicals will depend on a worker’s individual risk profile but will take place at least once a year. And when a person leaves his or her position, for whatever reason, an exit medical is a legal requirement.

A mining medical generally consists of:

  • Full physical examination, including
  • Eye and Audio tests
  • Blood and Urine tests
  • Chest X-ray
  • Drug strip test

We will also do biological monitoring if a risk assessment shows that a mineworker is, for example, exposed to lead.

The risks mineworkers are exposed to vary widely and will depend on the type of mine and the commodity that is being mined. All mines must report each year on the number of initial, periodical and exit examinations that have been conducted as part of their medical surveillance system. This report goes to the Mine Health and Safety Inspectorate and must be compiled by a certified Occupational Medical Practitioner (OMP).

Among other things, it should also include:

  • An analysis of the employees’ health based on their records of medical surveillance.
  • Comments on the future direction of the medical surveillance system.
  • The number of employees certified for compensation for occupational diseases.
  • The total number of employees, including contract workers, who were subjected to medical surveillance in terms of the Mine Health and Safety Act 1996, as amended, during the reporting period and the total of hours worked by those employees.

By law, cleaning, hygiene and recycling workers fall into three categories, all requiring a medical certificate of fitness:

  1. General – limited risk exposure to dust, noise or hazardous chemicals.
  2. Risk-Based – chemical exposures (cleaning agents), and dust.
  3. Operator – operators/ drivers of vehicles.
